How to get Ayushman Golden Card
- Open the link given below on the website of Ayushman Bharat Pradhan Mantri Jan Arogya Yojana-Â https://bis.pmjay.gov.in/BIS/selfprintCard
- In this, you have the option to select only Aadhaar as an option. Click on the circle just before it.
- As soon as you click on Aadhaar , 3 boxes open in front of you, in which you will have to fill some information.
- Scheme *: In this you have to select the option of PMJAY. (This option is available for normal citizens)
- Select State *: In this you have to select the name of your state, as we have selected UTTAR PRADESH.
- Aadhaar Number / Virtual ID *: Here you have to enter your 12 digit Aadhaar number. If you want to keep the Aadhaar number confidential, then you can enter Virtual ID .
- You have to tick the box like this. It is your consent that your Aadhaar number may be used as eKYC and the details entered in your Aadhaar may be taken for verification here. In English it is written like this- The beneficiary has no objection to provide Aadhaar Number for fetching eKYC (Electronic Know Your Customer) Details from UIDAI database for AB-NHPM.
- Generate OTPÂ : Finally, click on this button at the bottom.
- Enter OTP: An OTP number will be sent by SMS on the mobile number registered in your Aadhaar card. Drop it in the space provided. After this , verify by clicking on the button of Verify .
- The details entered in your Ayushman card will appear on the screen and there will also be an option to download it.
- Download your Ayushman card by clicking on Ayushman Card Download . You can also save it on your computer or mobile.
How to check your name in Ayushman Yojana?
Note: There is no separate process to register for Ayushman Bharat Scheme. But you can definitely check your name. Its process is as follows-
- Go to the website of Ayushman Bharat Yojana. Its link is-
https://www.pmjay.gov.in - On the homepage, you will see a question mark  (?)  logo on the right side, with a capital E (E) in the background. If you move the cursor on it,  ‘Am I Eligible’  will appear. Click on it.
- There will be two empty boxes in front of you. In the first you have to enter your mobile number and in the second the CAPTCHA code. After this click on Generate OTP  .
- 6 digit  OTP  number will come on your mobile. Put it in the OTP box and give consent by ticking the empty box before a declaration below. Then   click on Submit .
- Now select your state name from the dropdown list. Now you will get the option to check your name on the basis of some options. These options are-
- with the help of your name
- With the help of HHD number (HHD number or Household Id number is a unique number, which is given to families after the SECC census.)
- With the help of ration card number
- by mobile number
- If your family is included in the beneficiaries of Ayushman Bharat Scheme, then your name will appear in one of these ways.
